About ACH Forms

Free ACH forms, return code lookup and NACHA file tools for US small businesses.

What this site is

ACH Forms makes ACH paperwork easier. You can make an ACH authorization form, a vendor ACH form or a direct deposit form in a couple of minutes, look up what an ACH return code means, and check a NACHA file before you send it to your bank.

Everything is free. You do not need an account.

Who runs it

ACH Forms is built and run by EmproIT. You can reach us at [email protected].

Why it exists

ACH is how most US businesses get paid and pay their people, but the paperwork is confusing. Bank forms differ, the rules are long, and a return code such as R03 or R10 tells you very little on its own. We wanted one place where a small business owner, a bookkeeper or a payroll admin can get a correct form and a plain answer.

How the tools handle your information

  • The form builder and the file tools run in your browser. This site does not upload your details or your files.
  • Bank account and routing numbers are never saved, not even on your own device.
  • Your forms carry no watermark and no ACH Forms branding.

Read the details in our privacy page.

What this site is not

  • Not affiliated with Nacha. Nacha is the organization that writes the ACH Network rules. We are independent.
  • Not a bank or a payment processor. We do not move money and cannot see or trace your payments.
  • Not legal or financial advice. Our forms are built to include the elements Nacha rules ask for. Check with your bank before use.

How we keep it accurate

Every rule, code and file format detail on the site is checked against published sources, and each page lists them. See how we check our information. If you spot a mistake, please tell us.

How the site is paid for

The site shows ads from Adsterra, between the parts of a page and in the side column. Ads are never placed inside a tool, and they never change what a form says or what a code means. What the ads collect is on our privacy page.
